(If your provider has recommended that you see a specialist and you have made the appointment, you can fill out this form and return it to us to get your referral.)

As your primary care physician, we should decide when you need to be referred to a specialist. Please call our office at 978-251-3159 (and press option 3) to make an appointment with one of our providers who can evaluate your condition. In some cases you may not need an appointment with us first so please call for a phone consultation.

If we refer you to a specialist please call to make an appointment with them. After the appointment is set up we may need to send a referral for your insurance purposes. You can request a referral by contacting our referral specialist at the office or by mailing or faxing back to us the referral form you received at your office visit.

Please call at least three to five business day prior to your appointment. It is easiest to call as soon as you make your appointment with the specialist so that there is plenty of time to get the referral. Be sure you have the name of the specialist and date of your appointment before contacting us.
