Billing and Co-payments

Co-payments will be collected at the time of service.

Know the benefits and limitations of your insurance plan. 

It is your responsibility to talk with your insurance carrier and verify that we are in-network providers with your plan.

You are required to bring your insurance card with you to each appointment.

For contracted insurances plans, we will submit claims. Otherwise, payment is expected at the time of your appointment. 

Payment for non-covered services is required at the time of service.
